A Day in the Life of a Surgical Nurse

Being a surgical nurse is very different from other specialties, because surgical nurses deal with patients who are asleep. Surgical nurses see the patients very briefly in pre-op and then take them back to the operating room where they will be put to sleep by either an Anesthesiologist or a Certified Registered Nurse Anesthetist (CRNA).

Surgical nurses are very territorial and mysterious. No one else really knows what goes on behind those OR doors, (neither patients or other nurses ). It's a completely different world in surgery and without the proper training, you're not permitted to enter the surgical area.

Surgical nurses don't change dressings; they usually don't administer medications (except for local monitoring). They don't answer call lights or deal much with patients families. So what the heck do they do?

Well, behind those surgical doors are some exceptionally trained nurses who deserve recognition and praise, which is something they rarely get.

They don't see how a patient recovers. The patients are so high on Versed that they have amnesia after their whole surgical experience.

If they're on day shift they arrive at the hospital or facility around 6:00 am to be ready to set up a case at 7:00 am. This gives them time to change into scrubs and read their schedule. The schedule is their fate for the next 8-12 hours. They look at the big board by the front desk to find out if they're the scrub nurse that day, or the circulator. The main thing they're looking for on the board is which surgeon they'll be working with. This simple thing can make or break their day. There are both good and bad surgeons, just like any other slice of the population. "Please God, don't let it be such and such."

Surgeons can be friendly, but their skills may be horrible. Or they can be great Surgeons, but real jerks. Hopefully that day you will be assigned all the surgeons that are both friendly and good at what they do... but it isn't likely.

If you're assigned to be the circulating nurse, then you grab your scrub tech/nurse, and you both go to locate your first case cart of the day. This could be anywhere in the mess of other carts that have been filled with items needed for other cases. Hmmm, what a joy this is when you have a big ortho case and half of the instruments aren't sterile and need to be flashed Better yet, half of the items on the preference sheet are missing.

You have to run and find them while your scrub nurse is opening the sterile field. When you return you "dance with your scrub nurse". Not literally, but to "dance with your scrub nurse" actually means you help the scrub nurse tie her/his sterile gown. They can't do this on their own, or it would render them unsterile, for reaching behind their back.

You then must count everything, including all the instruments, raytec, laps, needles, and blades. Remember all this is done between 6:30 am and 7:00 am. Heaven forbid you lose a lap or any of the above items. It's a nightmare when you lose anything. I've been in cases where we were removing a lap sponge, a needle or an instrument; these cases are so much fun. During cases where the surgeon has previously left a sponge inside the patient, you definitely need a dab of wintergreen on your mask, or you are likely to puke your guts up! (and that's putting it lightly). Anyway, once everything is counted, your scrub nurse is happy, your OR bed is sheeted and all the equipment is in the room, it's time to go out and greet the patient.

You go to pre-op to introduce yourself to the patient and evaluate the chart. God only knows what crazy stuff you'll find in there. The labs may be way off, and the surgery may be canceled. The patient may be allergic to latex, so the whole sterile field has to be broken down, because you've already placed a latex foley on there. You walk into the room and address the patient in as cool a manner as you can, (trying to remember that this patient is scared out of their wits) unless they have had Versed. Such a wonderful drug!

Anesthesia has usually seen and evaluated the patient before you arrive, and the patient has already been asked 3 or 4 times whether they've had anything to eat or drink since midnight. But when you ask the patient the same question, all of a sudden their answer changes. They tell you all they had was a doughnut and coffee for breakfast that morning! Okay, so now the case is abruptly canceled and you're lucky enough to have the task of breaking the whole operating room down room down and starting over. One of numerous other scenarios may be that the patient is allergic to shellfish or peanuts, (which is the allergy de joure these days). Everyone and their mother has a peanut allergy. Or maybe, the patient is just allergic to their own snot!

Today the patient has none of these problems. They're not obese nor pregnant, so there'll be no need to pull out the Hercules bed. Hip hip hooray, the surgery will proceed. You begin wheeling her back to the OR after she's had her "margarita in a vial", (Versed), and before she tells everyone in the pre-op area every secret she has.

She goes on to talk your head silly all the way to the surgical suite, and she tells you how she'll never forget how wonderful you are. In your mind you're thinking Yeah, right, you won't remember your own name when you wake up, let alone mine. After entering the OR you transfer the patient onto the table and find that she's still wearing her underwear, (complete with latex banding), even though she told you she had a latex allergy... Awesome!

You assist the CRNA or Anesthesiologist to put her to sleep, (in a hurry, cause she is driving you nuts), with her "jabber, jabber won't shut up". CRNA or Anesthesiologist to put her to sleep, (in a hurry, cause she is driving you nuts), with her "jabber, jabber won't shut up".

Alas, she's asleep, and all is quiet for a few minutes, until in bursts Doctor Friendly. He's had a bad day doing rounds, and he's been paged 54 times by his office staff, so he's in a lovely mood, and you're in for a lovely day.

Nothing on the preference card is right, and you spend your time running around searching for instruments, (dirty ones, which need to be flashed). This only pisses the surgeon off more and enhances your day further. The bovie isn't working, and the Rad Tech has been called for a C-arm 10 times but is still MIA.

When everything begins to settle down and all the issues have been resolved you can relax for 5 minutes and sit quietly, hoping it stays that way. Finally the surgeon is closing and you begin counting. Laps and raytec first, followed by instruments, then needles. All are correct, (well except for one tiny needle) that is nowhere to be found. The scrub counts again. "No, still missing." The surgeon is about to knock someone's head off and freely verbalizes it. You run for the magnet on a stick to roll it on the floor and find the friggin needle. Finally, you find it next to the scrub nurse's foot.

The patient is beginning to rouse, and you are finished with the case. You transfer the patient to post-op and give the PACU nurse report. Yay, it's lunch time, and you're exhausted, with only five more cases to go.

This is a day in the life of a surgical nurse. Many nurses in other specialties believe that surgical nurses really don't do much or aren't "real nurses". While the surgical nurses role is very non-traditional, they work very hard and they're an integral part of the nursing profession. Unfortunately, they don't get to see the fruits of their labor. Once the surgery is over they never see that patient again and usually have no idea how well the patient did in their recovery. The patient doesn't remember the great care they got from all the OR staff and for the patient's sake, it's probably just as well.

No Gym, No Problem - Affordable Home Gym Ideas

If you have ever lived in an apartment and you're anything like me, then it is almost torturous to have the same layout for the entire time. Unfortunately you are locked into a lease and you have to make the best of it. Even if you loved your place when you moved in you could get bored of the way your things are set up. That being said, I wish my girlfriend and I had a second bedroom so we could put all our workout stuff in. Yes we could just go down to the fitness center in our apartment complex but honestly I feel a little odd due to its close proximity to the leasing center. Going to a gym is one thing but I like to work out when I can upstairs.

A few things that I have found to be very useful in working out in my apartment are resistance bands, a pull up bar and a punching bag of any weight. The resistance bands offer incredible versatility because you can use them as nearly any piece of gym equipment. For instance you stand on them and curl or wrap your back in them and do push ups with added resistance. You can use them in almost any way and get a good work out going.

The pull-up bar is used of course for pull ups but you can take the punching bag, hang it from your pull up bar and do some light kicks and punches. I used to have a punching bag stand with a 100 pound bag and I loved punching it and kicking it. It was one of the best workouts I have ever done and I was very disappointed when I got rid of the bag and stand. The punching bag can be used as other things too. You can bench it or squat with it. Almost anything you do with weights or without weights can be done with this punching bag. It is a little cumbersome even if it is a light weight bag. You can train your auxiliary muscles this way too. Due to the awkward nature of working with the bag your body needs to stay straight and stable with.

These options have really made it easier to work out at home and It is a very affordable option, the bag was just about $35, you can get a pull up bar for $10-15 and the resistance bands cost me about $18 on amazon. So it is a very viable option if you don't want to spend the money on a pricey gym membership or if you just don't like working out in public settings.

The question we are asking ourselves here is whether our own personal life, in its current state, is designed to support wellness.

This requires a dual approach.

We must investigate exactly what wellness is referring to.

Does it mean a general state of well-being?

Can we be well physically but not mentally?

Does this have anything to with spiritual wellness?

We must recognize whether or not wellness is suited to our life currently.

Maybe we already live in a way that promotes wellness in our everyday life and maybe we do not. However, in either case, we want to understand how well our life is suited to wellness.

For those of us who do not live in a way that promotes wellness, what do we have to do to inspire wellness in our current life?

Do we have to make any changes or is it simply a case of doing more?

What is Wellness?

Wellness is something that many people overlook. Most people think that how they feel is how they feel and there is nothing to be done about it.

For some, the primary goal of seeking good health is to prevent some possible future disease, like heart disease or diabetes.

Some others relate wellness only to the physical body and others relate it more to their mental state than anything else does.

Let us have a look at a holistic view of what wellness is. Quite simply, wellness can be understood as balance.

Wellness = Balance

When we feel well physically it is due to the fact of our bodies is getting what they need from a balanced lifestyle. This includes our level of physical exercise, our nutrition intake, our level of work vs play, and our personal and environmental hygiene. When we feel balanced physically, we feel well.

Mentally we feel balanced when our thoughts are aligned with our purpose in life. When we have an understanding as to what we are doing, why we are doing it and what needs to be done to maintain order in our life. When we are emotionally disturbed on the other-hand, we say that we feel out-of-balance, or perhaps mentally unwell.

Spiritually, we have a much bigger picture to look at. In order to determine whether we are spiritually well we need to be very honest with ourselves when asking that question.

If we feel completely happy with our lives, if we have a genuine understanding of our function and position in the universe, as living beings, then we could be said to be spiritually well. But, and this is where the importance of self-honesty shines through, we must be clear with ourselves if we feel spiritually unwell.

Do we feel confused about our way of life, or our beliefs?

Do we have unresolved conflicts with our past or even perhaps our understanding of our life, or our purpose?

Do we feel inner calm and peace that supports and promotes our utmost quality of life?

Being spiritually unwell means that no matter what we do, or where we go, or who we are with, our happiness, our wellness is only temporary.

Eventually we always fall back into some form of despair or confusion, or suffering: like having a splinter in our minds that inevitably, always returns back to hurt.

The Best Thing About Wellness

The best thing about wellness though is that it is attainable, by anyone and everyone. Everyone and anyone on the face of the planet can achieve physical, mental, and spiritual wellness. Bearing in mind, that wellness does not mean 'to be in good health'. If that were the case then no terminally, ill person could ever feel well.

This is what is meant by, wellness = balance. Even a person who is suffering from a terminal illness can feel well, if their life is balanced and fulfilled.

In order to encourage the development of a holistic wellness in our life, all we have to do is maintain a balanced way of life. This incorporates the physical, mental and spiritual worlds, since all of those things are within us.

Therefore, with regards to the question, is your lifestyle designed to support wellness, the above will give you an idea if it is, and only you will know the answer. If the answer is no, there are many things you can do to make changes, so make a plan and stick with it.

Feeling Better About the Day - How to Wake Up Early in the Morning

Typically, the alarm clock rings and the struggle between your willpower and that horrible sound that forces you out of bed begins. Which ruins any attempt to get up early again.

For life, I have been part of those who pray for "5 more minutes in the bed" and multiply 20 times postponing the alarm again and again. At last, wake up and get out of bed like a zombie (with the feeling of not having enough rest).

But there are studies and research that conclude that getting up earlier, around 5:30 in the morning, has its great benefits. Here is a summary of the benefits of getting up early and having a ritual in the morning, plus the best strategies for getting you to wake up earlier.

These are the advantages of being a "morning person."

Research suggests that if there are biological differences between "people of tomorrow," they wake up at the same time every day and feel more active around 9 am, and "night people" who manage to do more things once That the sun is hiding.

However, a point for the morning people is that they can get better effectiveness and quality of work waking up early, unlike the "night owls." According to self-reports from college students, those who wake up earlier feel more optimistic, proactive and have better grades. Other studies have found that morning people tend to be more productive and focused than night owls. Researchers also suggest that memory and concentration during the day improve when you sleep.

Physical impact

Being a morning person can be really good for your health, and even if you do not believe, it can be good at controlling your weight. When British researchers questioned some adults about their sleep habits, they found that people who stay under the sheets for a longer time are more likely to live with stress, overweight, and depression than those who wake up at 7 a.m.

Another study found that waking up late makes you more prone to being sedentary and therefore being overweight, compared to those who wake up early. Also, stopping earlier is more advantageous for your schedules and your daily routine. Usually, the working day usually starts around 9 in the morning, and the office (usually) will not open at midnight.

The problem to get up early is not in your mornings.

Enough sleep means sleeping on average 7 to 8 hours a day. It is now known that this number decreases as age increases. This occurs due to changes in circadian cycles, better known as biological clocks.

If you are part of the average adult group, it means that there is no reason to sleep less than 7 hours a day. It is also very likely that this will also help you get up early without being sleepy (and even without an alarm clock).

How can I get up earlier?

Remember that the most important thing is to incorporate new habits, and to do this you have to follow the steps. Once you have mastered the first, you can continue to the next and so on. Before you start these steps, you need to visualize your ideal tomorrow.

This is important so that you always keep in mind the real reason why you want to wake up early, which motivates you to be consistent. Also, you can have a ritual in the morning in which you do the following, helping you to create the habit.

After having your coffee, you close your eyes, meditate and spend five minutes to thank for life

Write positive affirmations of why you love to get up early and how great your day will be

Visualize and attract the life you dream

Exercise a little

Read a few minutes, books to help you grow

More tips for waking up early

Sleep earlier. Use an alarm that reminds you when to go to bed so you can rest for at least seven hours.

Set your alarm five minutes early. Setting your alarm 5 minutes earlier or even just 2 minutes earlier generates a sense of triumph, which motivates you to continue with larger goals, such as waking up 15 or 30 minutes earlier.

Lift up as soon as the alarm sounds. Setting the alarm earlier is only meant to be. Because the real challenge begins, when you have to get out of bed without postponing that alarm over and over again. Everything here is a matter of will.

Do something to reward yourself for getting out of bed like the ritual we showed you earlier.

Keep a record. For it to become a habit, you have to wake up at the agreed time every day (re-read that phrase). One way to know if you have succeeded or not is simply to keep a daily log.

Just dare, you will surely see the changes and benefits of creating the habit of getting up early. I personally loved it, and I feel more productive now.

Do You Know What Cupping Colour Means?

Cupping has become very topical since Michael Phelps showed off his cupping marks at the 2016 Olympics Games in Rio. Of course, cupping is a very ancient form of Chinese medicine that has been around for thousands of years and has been used by various cultures all over the world. Historians believe that cupping is, in fact, the most ancient technique in Chinese medicine, predating acupuncture.

What is Cupping?

Suction is applied to the skin using glass, rubber, or plastic cups. In ancient times, doctors used bamboo. The vacuum action rapidly facilitates the release of rigid soft tissues. It loosens and lifts connective tissues, it breaks up and drains stagnation, and increases blood and lymph flow to skin, muscles, and connective tissues. It does all of these things far more rapidly and effectively than any other type of therapy, including acupuncture, massage therapy, chiropractic adjustments, medications and sometimes even surgery.

What do the Colours Mean?

The colour change on the skin after cupping indicates many different things. Sometimes it turns orange or purple and, for others, it may turn red. The marks usually last from three days to a week; sometimes longer if a person is very sick or sedentary. The colour change on the skin after cupping also helps to determine the nature and the location of an illness but, for a healthy person with healthy skin, the tissue colour does not change at all after the cupping.

Since you can use the suction cupping almost anywhere on the body surface except in areas where large blood vessels pass, such as in the groin, you can check to see if you have internal troubles from the outside of the body. In areas where there is dead blood, lymph, cellular debris, pathogenic factors, and toxins present in the body, cupping will leave marks which indicate that the stagnation has been moved from the deeper tissue layers to the surface. The colour and pattern of the marks depends on the level of stagnation in the area. If there is no stagnation present, there will be only a light pink mark which disappears in a few minutes to a couple of hours.

Sites where there is old trauma or injury may require multiple cupping treatments to remove all stagnation. Patients will find, in follow-up treatments, that the marks will be lighter and lighter as the pathogens are systemically removed from the body. Cupping marks are not bruises and they are generally not painful. Although the cupping treatment itself can be slightly uncomfortable and the marks can look dramatic, the presence of colour in the cupped areas guarantees that the patient will feel immediate relief after the treatment.

Who Should Do Cupping?

Although cupping is a simple technique, it should only be performed by an expert practitioner. Although some massage therapists, chiropractors, or estheticians offer cupping therapy, they generally lack the in-depth training and experience that is required to apply cupping therapy safely and effectively. At best, cupping therapy performed by an untrained practitioner will be ineffective. At worst, it can cause burns, blisters, or scar tissue that result in further pain as opposed to providing relief.

If a meditator or tapper, you can do your meditation or tapping in the morning as soon as you wake up. Or you can simply enjoy watching the dawn evolve as you hear life outside slowly awakening.

To be able to wake up early, you of course need to sleep early enough. This depends on your day, so for example, if every Tuesday evening, you have a personal trainer or a Yoga class or a Reiki share, and you only then get home for 10 pm, then you will need to go to bed almost straight away. This takes some planning. For example, being able to have your supper early somehow. Most people generally agree it is not ideal to eat a heavy meal just before going to sleep.

So, how can we go to sleep earlier?

I like using EFT. Short for Emotional Freedom Techniques, otherwise known as Tapping, EFT can help you to relax into sleep. Tapping for "I happily go to bed at 10 pm" for example, can help release any objections of your subconscious mind. You may have to do this for a few weeks for the full effect to take place, but it takes only minutes, so it is easy to do. For some, sleep is always a problem, due to getting up frequently as a light sleeper. This is also where EFT can help. EFT can release the stuck energy imprints left in your mind-body from negative events that you probably think you have dealt with and are no longer affecting you. However, their cumulative effect may still be lurking in your mind-body, causing lighter and lighter sleep. Nutrition is also important, and it is good to consult an experienced functional nutritionist to ensure that any nutritional needs for sleep are being addressed. Hygiene in Nineteenth Century America

In August 1879 a young Robert Louis Stevenson boarded the immigrant train in New York, bound for Chicago. The middle class Scot was journeying to meet his American true love, Fanny Osborne. His parents, unamused by the union, refused to fund his journey. In love but suddenly impecunious, the young author was forced to travel in economy class. His daily life for the next few weeks is one that most nineteenth century Americans would have recognised.

This is a world, Robert chronicled, that was intimately communal. He soon became accustomed each morning to unfurling his stiff limbs from the bench next to his fellow travellers. His ears became attuned to the dawn chorus of coughing, sneezing and spitting rippling down the carriage. His educated brain tried to suppress thoughts of cholera, tuberculosis and diphtheria. An enclosed environment such as this was the perfect breeding ground for these and other potentially lethal diseases.

The 'rancid stench,' as he graphically put it, of his fellow travellers was impossible to ignore. Like everyone else, Robert wore the same clothes for days. The cowboy could blend with the smell of animal and leather, but on a train there was nowhere to hide.
He could at least cleanse the body. But these were the days before hot piping water at the twist of a tap. All water onboard had to be stored and drawn from a water filter in one corner of the carriage. A hot bath was a luxury most Americans experienced once a month and one Robert would enjoy at his journey's end. Until then a wash meant a perfunctory sluice using soap, flannel and tin wash basin bought from the newsboy onboard. Once refreshed, these utensils would be recycled by his fellow travellers. For those without much, sharing was a common currency.

No one, not even the rich and powerful in First Class, could disguise bodily odours with the deodorants we take for granted today. Ancient Egyptian women are said to have poured globules of perfumed wax over their heads, or applied porridge to the more sensitive areas of their body. These and other exotic remedies were lost to history. Robert and most Americans would have to wait until 1888 before they could dab on the first commercial deodorant.

Before returning to his seat, Robert might run a comb through his matted hair. Washing out the lice and grime could wait until he found a barber. The ladies on board would also bide their time. Brushed hair was more important; the monthly hair wash, perhaps with one of the available powder shampoo and conditioner, could wait until they arrived at their destination. No one rushed to use the available bar soap made of Sodium Hydroxide. This could be very hard on the hair and needed something equally basic, such as vinegar, to rinse it out.

Settling back into his seat, the young Scot might engage in conversation with his neighbours. This, he quickly discovered, could be hazardous. Oral hygiene was not a high priority in the nineteenth century. Fresh breath, clean gums and sparkling white teeth, much prized in Ancient Greece and Rome, were not common.

They might have washed their mouth out with baking soda, salt or even soap. If Robert was very lucky, they used a tooth powder like the one advertised in the Dodge City Times of 1879, containing, amongst other things, charcoal and bark. As would the cowboy by the campfire, running a cursory finger across his gums with cold ash from the embers. Robert, like us, might have recoiled at the thought of sharing the communal toothbrush available at public eateries and wagon stations. Everyone prayed fervently that they didn't need a tooth removed. In a world that was totally unregulated, where dentists didn't exist, that meant going in search of tradesmen with the means to extract. Your friendly barber, for example.

One bodily function Robert shared with no one was relief of his bowels. Onboard toilet facilities did not become available until the end of the nineteenth century. He, like everyone else, had to hold on until the train stopped. As they alighted from the train, they were advised to hold their nose.The stench from waste, human and otherwise, would have been appalling. The cowboy in the wilds of Texas or Montana enjoyed cleaner air. It is no accident that the Native Indians regularly moved camp. They weren't merely following the Bison. They were escaping the human detritus that accumulates in any human encampment.

A bustling city like Chicago could boast the latest mains sewerage system. But Robert and his contemporaries would have to wait until 1907 before sampling the first flush toilet. The cowboy could simply disappear into the bushes with a corn cob or clump of grass. Young Robert perhaps sampled Joseph C. Gayetty's packaged toilet paper, pre-moistened sheets medicated with aloe. More likely he recycled a few sheets torn from a Sears catalogue - perhaps a few sheets of the humorous spin-off, 'Rears and Sorbutt'.

There is an old and tired idea about not using drugs to treat drugs but it is better than burying so many until we figure out an answer. At least new meds can keep some alive while we come up with something better than a treatment written in 1939 that hasn't changed a word since.

It is time to try to bring the new drugs into use and safely start human trials. If Europe is having success with Baclofen, China with Kudzu, Africa with Ibogaine, why couldn't we be open to new ideas as soon as possible?
