The Problems With the TSB Rollout Published


Since at least April, IBM has been looking at the issues surrounding the shocking TSB software rollout that cause so much banking pain for customers.

All in all it looks like the War Room's would find a problem which was then changed without checking with other War rooms. It seems shocking to this writer that any software organisation with so many customers and especially in the finance sector would not have a plan to roll back if there was a failure, never mind the simple mindedness of TSB actually believing that they had tested everything prior to putting the system "live" for the world to see (crashed and burned).

The IBM presentation document makes interesting reading with several hints for software houses on IBM's own practices. All of the stages they mention and highlight as TSB failures along with the recommendations should help anyone write a comprehensive playbook for testing and rolling out new software.

This ex-software test engineer is please to see IBM pointing out so many errors that can be learnt from by others. I seem to recall the MD of a company I used to work for stating "We don't need a test department, that's what customers are for". As shocking as that is to anyone in the sector it seems that TSB's test team have forgotten some simple lessons and should go back to the drawing board.

At some point you have to realise that clear communication between the Software Team, the Test Team and Management are essential, in the case of TSB I would have to guess at a communications breakdown.

For TSB customers, the nightmare has been very real, for TSB it has led to more bad banking press and that has to generate trust issues. Lets remember that publishing faulty code that causes systems to grind to a halt is never great, but think of the potential for vulnerabilities in the system and suddenly I'm glad I don't bank with TSB!

Error 651 Windows 10 Broadband Connection Fix

The 651 error is caused by a fault or incompatible network card / modem being unable to connect to the Internet or Local Area Network.

The issue is software related (not hardware) and is therefore relatively simple to fix, if you're able to pinpoint the exact cause of it. This is - unfortunately - the thing that prevents most people from being able to resolve it properly... getting to the bottom of the error means that you have to try a number of other fixes.

The error mostly shows with the following message:

"Error 651: The modem (or other connecting device) has reported an error"

Cause

The main reason for the error to show is driver incompatibility.

Windows uses "drivers" to help it communicate with a number of different pieces of hardware, and are an essential element in the way the system works.

Unfortunately, the drivers that your system has will often become corrupted, damaged or out of date - leading various elements of a system's functionality to become unusable or erroneous.

Whilst the error is quite old, the best solution is to re-install the driver then ensure that other aspects of your system are working as effectively as possible (IE not blocking the ethernet connection).

Solution

To fix the error, you first need to ensure that the driver is working properly, and then that you're able to repair any of the specific causes of the problem (several files are known to present issues).

1. Update / Reinstall Driver

The first step is to update or re-install the driver for your ethernet card / modem.

To do this, you should restart into "Safe Mode with Networking" and then replace the driver:

Click the "Start" button at the bottom left of the taskbar

Select the "power" button from the left "charms" menu (icons)

Holding SHIFT, select "Restart"

This will open a blue Window from which you need to select "Troubleshoot" > "Advanced Options" > "Startup Settings" > "Restart"

From the next Window, select "Safe Mode"

This will reboot the system into "Safe Mode" - basically allowing you to perform core system maintenance without third party software getting in the way.

Right-click the "Start" button (bottom left of taskbar)

Select "Device Manager"

Scroll down to "Network Adapters"

Right-click on the card which is causing issues and select "Uninstall"

Follow the instructions on-screen

This will get rid of the hardware listing from your system - allowing you to manage the driver as you need.

From another system, you'll then need to download the correct driver from the manufacturer's website. Whilst you can get away with just restarting Windows (and letting it run the installation), it may put the wrong driver back on:

Save the new driver to another system

Use a USB flash drive to copy the driver to your damaged system

Right-Click the card in "Device Manager" and pick "Update Driver Software"

This will present you with two options - select the "Manual" one and choose the files you just copied via USB

After doing this, restart your PC into "normal" mode (just restart the system as you would normally).

2. Replace "rasppoe.sys"

Rasppope.sys is a file used to provide PPoE protocol functionality to Windows.

Located in C:/Windows/System32/Drivers, the file is used to provide PPoE connectivity, be it a modem or network card.

One of the problems with Windows 10 is this file becoming corrupted, incompatible or overwritten - preventing your system from being able to run the functionality required.

To fix this, you need to rename & replace it:

Open Windows' "File Explorer" (Start > left "charms" menu)
Browse to C:/Windows/System32/Drivers
Locate rasppoe.sys
Rename to rasppoe_bk.sys
Download a new version of rasppoe.sys from the web (Google "rasppoe.dll download" - look for "OpenDLL (.) com")
Place the new file in C:/Windows/System32/Drivers
Restart PC

If the error still persists after this, you will also need to run Windows Update, run an antivirus scan and then remove any potentially conflicting software from your system.

Document Automation or Document Risk Mitigation?

Document automation, document assembly, and document generation - these are all ways of describing the process of using templates to create error free documentation.

But are any of these the most accurate description of what the software actually does?

Different industry verticals identify different strategic advantages in document automation software. Organisations such as banks or large legal firms often use document automation technology to reduce risk by minimising human interaction with the documentation.

Legal documents, such as contracts, insurance forms, wills, etc. are notoriously unstable-every time a governing body meets, the laws governing the documents could change. This reality-the instability of legal documents-gave way to platforms designed specifically to automate legal documentation.

When it comes to banks or financial institutions, the main value proposition that attracts them to document automation software is better documentation (and again, less risk). Every institution in the sector-from global banks to local agricultural credit unions-has the same problem: non-legal experts generating and executing binding contracts worth a significant amount of money. Consequently, banks need a document assembly platform that can generate complex, business-ready contracts. ExceptionHandler Ruby on Rails Gem

ExceptionHandler is an "error pages" gem for Ruby on Rails.

Currently at version 0.7.7.0, the gem has been downloaded over 170,000 times and is widely regarded as the "best" dynamic error pages gem for the framework. It works extremely well.

The most important thing to realize about ExceptionHandler is that it's basically designed to provide a "translation" system for Rails errors, converting them into the appropriate HTTP error that a web browser can read.

You see, whilst Rails may raise exceptions from within its core, these errors are not what you see in your browser. You see the http error that the framework delivers to the web server. This http error is accompanied by an "HTTP Message Body" which is what the browser displays on the page.

In other words, when dealing with Rails errors - if you want to actually show "branded" error pages (with your own layout or another one) - you have to "hack" Rails to deliver those specific pages when errors occur. The pages that are shown have NO bearing on what the user sees in their web browser; they're simply there to provide a "branded" way for them to engage with it.

Whenever Rails raises an exception inside its application - that error (it could be anything such as a database problem or something) is only valid for the Rails application. What Rails does is "translate" that error into one of two types of HTTP error (which a browser can read): 4xx (client error) or 5xx (server error).

Every time you use a "web" application, your computer is sending a request for data on port 80 of a publicly accessible IP address. If the other computer (server) has a "web server" application running on port 80, it will accept the request and respond with HTTP based data (which will typically include HTML code).

The entire "web" is a public directory for the "Internet", meaning that if you the IP (or equivalent domain name) for a connected computer, you should be able to access it via the "HTTP" (Hyper Text Transfer Protocol). This HTTP protocol is the core of how the "web" works, and why most people get confused when dealing with "errors" in their Rails based applications.

HTTP "errors" are not really errors at all but erroneous responses. Each "error" you see is still an HTML page, displayed with a corresponding HTTP status code.

Since HTTP is a "stateless" protocol, it has to work with what's known as a "request/response" pattern - essentially meaning that every single time you send a new request to a web service, that request is treated as entirely new.

This is opposed to "stateful" processes, which "retain state" between requests; in the sense of native applications or similar. The point is that what you see with HTTP errors is a response to an erroneous request. They're really just status codes which explain that.

To do this properly, you have to be able to "translate" Rails-based errors into the appropriate HTTP error response. This is done by the ActionDispatch::ShowExceptions middleware - which calls a middleware "hook" ("exceptions_app") to determine which "HTML" response to show in the body of the erroneous message.

It uses the "rescue_responses" hash to determine which HTTP error code to match any Rails exceptions to. This hash can be extended within Rails, allowing users to map different Rails-based "exceptions" to the appropriate HTTP response code.

Say your application raises an error with ActiveRecord.

If it cannot find a particular item in the database - Rails will raise the ActiveRecord::NotFound exception class. This is not an HTTP-compatible error; it's simply a "Rails" exception. The key is that it has to be "translated" into an error that HTTP-based browsers can understand. This is where ActionDispatch::ShowExceptions comes in.

ShowExceptions essentially takes the exception object - passes it through the "rescue_responses" hash (to get an appropriate HTTP status code) and - crucially - invokes the "exceptions_app" middleware hook to generate the "HTTP message body" for the response...

The key thing to realize here is that @exceptions_app is what ExceptionHandler has been built to manage.

Every time Rails raises an exception, the HTTP protocol still remains no matter what. In other words, you're always going to require sending an HTTP status code and message body to the requesting web browser... the difference lies in what you send.

The problem for Rails is that @exceptions_app defaults to the "routes" - which means that it will load 404.HTML or 500.HTML from the /public directory of your application. Whilst this works, the pages are unprofessional and static.

To change the pages, ExceptionHandler overrides the "@exceptions_app" hook with its own controller and views. This means that you're basically able to invoke "dynamic" web pages with Rails.

The way ExceptionHandler works is to use the application's "default" layout (typically "application") for 4xx errors, and include a completely custom "exception" layout for 5xx errors. The custom layout is generally recommended as 5xx errors denote server issues, which means that if you're using a layout that references the database (as most "application" layouts do), it could cause an infinite loop to occur.

This means that if you have an error - say ActiveRecord cannot load a record from the database - Rails will still invoke ActiveDispatch::ShowExceptions.

However, due to ExceptionHandler overriding the "exceptions_app" hook, rather than the static 404.HTML / 500.HTML pages being called, the controller and views pipeline provided by the gem are invoked instead.
