Getting Through It: Mindfulness Meditation for Pain Relief

Surgery was yesterday. The pain isn't too bad. It's tolerable, not as bad as I expected; yet, the pain still dictates my movement, actually, lack it. What can I do besides medication?

I found mindfulness meditation almost perfect. I use the word almost because any meditation takes practice. What I liked about this particular process was it was fast and easy.

The most it takes me is one or two minutes. I can even complete this at a stop light or in a grocery store line.

The purpose of this meditation, actually all meditation, is to focus on your breathing while keeping your mind open and clear. Different from the type of meditation I previously learned and found difficult to complete; where I needed a comfortable position, took more time and required me to learn to blank my mind. In this process you allow your mind to feel the pain or be open to whatever is playing in your thoughts. Just allow the flow.

The steps are easy.

Like any meditation practice, you want to relax any tension. Begin with one body section or one the muscle at a time. It doesn't matter which part and doesn't necessarily need to begin with the place of pain. Begin with your shoulders or maybe your back. You begin wherever emerges in your mind first. Feel the tension there and say in your mind, "go away now" and watch the tension release immediately.

After my foot surgery three months earlier, while I was at a stop light, I would briefly close my eyes, yes, briefly, picture my feet dangling off the side of my bed before touching the floor, began with wiggling a toe progressing up to my ankle -- methodologically. If the light changed I postponed to the next light and continued or start again. Eventually, as Tony Robbins, the motivational trainer, and a friend, also known as Anthony Robbins, taught me and millions of others, on how to create a "whoosh" feeling for this to dissipate within a split second -- one whoosh, the flash, and the immediate release. Yep, all gone in a second.

Accept any thoughts, feelings, and/or emotions that enter your mind. Don't push something up or back. Just allow and accept what is there. Sometimes when I'm driving I write them on a cloud while stopped if driving or more easily while riding and watch it drift away.

By now you may be thinking this may work just as well for anything painful going on in your life, not just physical pain and you would be right. It does. This works especially well when I write it on a cloud and allow the breeze take it away.

Like all meditation, breathing is important. You do want to notice how you are breathing, more so before you begin, but not necessarily during. Pay attention to the rise and fall of your chest or belly, not necessarily both. This doesn't take but another second or two.

Practicing mindfulness meditation and using it during a painful event can make a difference on your recovery as well. Even when I get a pedicure and she is digging into a hang nail I use this practice; however, I do recommend using the baby teething numbing medication to aid in the pain.

The Anxiety Opportunity

It seems to hold true that western society in particular is experiencing a significant amount of depression and anxiety. Whilst third world countries suffer with physical health problems, mental health appears to be the weak point in wealthier countries. It's easy to blame westerners for being entitled softies but the correlation between material security and increased anxiety is significant; I believe it signals an emergence from problem-solving at the survival level to problem-solving at the personal development level.

Let's start by defining anxiety as the anticipation of pain and suffering. Anxiety has three core aspects:



Thoughts

Emotions

Sensations

Most people who experience anxiety are very conscious of the emotional aspect and anxiety is often considered an exclusively emotional experience. However, under close observation it becomes apparent that the trigger for the emotional part of anxiety is preceding thoughts - ideas and beliefs that the near or distant future holds the promise of pain. We've all experienced this when we're told the boss wants to see us and we think we we're in trouble as we anticipate an unpleasant consequence. The thoughts, feelings and sensations we have leading up to that moment is anxiety.

Sometimes feeling anxious is valid; our expectation of future suffering is accurate and anxiety is the perfect preparation for handling a problem because it prepares our body to 'fight' or 'flee'. The biochemistry behind anxiety creates oxygenated muscles, a slowed-down digestive system (that sick feeling) and a hyper-vigilance to the environment which is useful for fighting or running, all at the expense of thinking logically which would slow-down our reaction times. Anxiety typically motivates us to avoid pain and suffering which, from an evolutionary point of view is highly adaptive. If we're walking through the woods and believe we notice a bear up ahead, avoiding the potential suffering is the best approach to take - skedaddle!

However, sometimes our anxiety is invalid because the problem doesn't exist, or the problem is being exaggerated and we overreact to the perceived threat. When this overreaction becomes regular and hard to control we have dysfunctional anxiety; this means the anxiety reduces our functioning rather than enhances it.

Typically, people hope for the perceived threat to diminish and in the meantime, metaphorically they hide in the bushes waiting for the imaginary bear to pass. However, when we use this method to respond to exaggerated unrealistic fears it has the effect of increasing our anxiety and, over time, reducing our skills for dealing with problems as we avoid them rather than face them and miss-out on the practice we require for handling difficult situations. Think of it like this: you can either hope the problem gets weaker or try to get stronger - most people opt for option one.

Those who are smart and courageous try the second option; they invest in becoming stronger so they have the capacity to handle more rather than hoping that life demands less. These people reap the rewards of improving themselves, they see life is an opportunity to learn more, be more and have more. There's no guarantee that you won't experience worry and anxiety, but people who face their fears are always the ones we admire the most.

If you can reframe the experience of anxiety (when there is no real threat) as a signal that you currently lack the skills to handle something, it can be the perfect invitation to growth. If I feel uncomfortable disagreeing with my boss, perhaps I need to learn some negotiation skills and book-in for a weekend workshop. If I regularly avoid thinking about money because it always makes me feel stressed, perhaps a serious visit to a financial advisor is in order and downloading some budgeting software would be the kindest thing I can do for myself.

For those of us in the relatively safe and luxurious west, anxiety is unlikely to be a true indication of impending physical harm, rather than an exaggeration of some more practical problem we are failing to solve. In this light, you can take the opportunity to combine anxiety with courage and self-care and transmute it into personal growth.

When the pus starts oozing out, slowly put pressure on the sides and move it upwards.

Make sure that your hands are also sterilized and the surrounding skin does not touch the pus of a pimple. An ideal way is to keep on cleaning the pus with cotton soaked in hot water.

After the entire pus flows out, apply an antibiotic ointment that helps to a dry the pimple and protects your skin from further infection.

Restrict or Liberalize? Therapeutic Diets for Older Adults With Cardiovascular Disease

Richard is an 88 year old male who was hospitalized post myocardial infarction (MI) for placement of a stent and management of heart failure. Other than a diagnosis of hypertension which has been under control with medications since age 70, he had been healthy and living independently with his wife in their home. Prior to the MI, he was eating a regular diet and either walking or using a stationary bike daily. However since the MI, his cardiologist has drastically restricted his physical activity, and ordered a low fat, low cholesterol, 2 gram sodium diet. His medications include 75 mg atenolol daily, 40 mg Lasix twice a day, 80 mg Lipitor daily, 8 mg warfarin daily, 75 mg Clopidogrel daily, and 80 mg low dose aspirin daily.

At 5'5" tall, normally 145#, he has gradually lost weight and is now down to 106#. Although his food intake is only fair, his cardiologist emphasized the need for a low sodium diet and daily weights to monitor for fluid shifts. His wife is overwhelmed and his daughter is nervous about how to shop and prepare for his meals.

Knowing that almost 20% of older adults are readmitted to the hospital within 30 days of discharge (1), assuring Richard receives good advice is essential. He will be returning home with his 88-year old wife, who is overwhelmed and nervous about preparing his food and with limited help from family. How should you counsel Richard?

Malnutrition Concerns

Medical nutrition therapy (MNT) recommendations for cardiovascular disease (CVD) in older adults vary based on diagnosis, age and risk of malnutrition. For frail older adults, unintended weight loss, malnutrition, sarcopenia, and frailty are genuine concerns. These conditions can lead to reduced functional ability and increased dependence, so preventing these complications is critical. Consuming nourishing food to prevent further decline is a major focus.

Health care providers mean well by ordering therapeutic diets to improve health, but in frail elderly, these diets can have a negative effect on the variety, flavor, and palatability of food. This can reduce enjoyment of eating, decrease food intake and lead to complications that could affect quality of life and longevity. It is critical to individualize nutrition interventions to assure the highest level of well-being for each person, and in many cases that means avoiding strict therapeutic diets. According to the Academy of Nutrition and Dietetics (Academy) Evidence Analysis Library, more liberal diets are associated with increased food and beverage intake in older adults (2).

Medical Nutrition Therapy for Cardiovascular Disease in Older Adults

A risk factor for cardiovascular disease, hypertension affects approximately 64-78.5% of older adults (3). Lowering blood pressure can help reduce risk for stroke, MI, heart failure, and renal disease. For people 60 years or older, blood pressure (BP) goals are <150 systolic and <90 diastolic BP (<140 and <90 for people with diabetes and/or CDK). All adults with hypertension should modify their lifestyles in conjunction with pharmacological treatment (4). To maintain nutritional status, older adults may need a more liberal approach to sodium restriction, especially if they are already frail (5).

Heart failure (HF) is the leading cause of hospitalization among older adults in the U.S., and more than 50% of HF patients are readmitted within 6 months of hospital discharge (6). Treatment includes medications, a reduced sodium diet, and daily physical activity (7). A 2000 mg sodium, 2000 mL fluid restriction is typically prescribed (8), however recent evidence indicates that intake of 2700-3000 mg sodium daily may decrease hospital readmissions and mortality in patients with compensated CHF (9).

Current guidelines for older with atherosclerotic heart disease recommend a focus on overall risk factors rather than specific parameters for blood lipid levels (10). It is unclear whether or not modification of blood lipids is effective to prevent CVD in older adults (11). Lipid levels can be controlled by medications while allowing an older person to enjoy food.

How Should Richard be Counseled?

In Richard's case, dietary changes should be balanced with his condition, prognosis, threat of malnutrition and cardiac cachexia, and individual food preferences. Richard should be counselled on eating healthfully to help maintain his health and focus on enjoying his meals. The Dietary Guidelines for Americans (DGA), the Dietary Approaches to Stop Hypertension (DASH) eating pattern, or a Mediterranean style diet are all good eating patterns for Richard. If Richard's food intake is poor or he is resistant to these suggestions, a more individualized eating pattern that includes his favorite foods may be needed, even if those favorites are high in sodium or fat.

The Bottom Line for Older Adults with CVD

It is important to assure person directed choice when working with older adults. Decisions should be informed and in coordination with the individual's goals, preferences and choices. Most older adults with CVD can benefit from a diet based on the DGA which is moderate in fat, cholesterol, sodium, and added sugars. Clean Eating Can Reduce Inflammation

After a recent talk I gave - this question was asked "How does inflammation lead to chronic disease?"

Chronic inflammation is a long-term physiologic response - lasting weeks, months, or years - to one or more factors, including exposure to environmental toxins, microbial or viral infection, processes related to aging, or stress, but most often to poor nutrition.

Chronic inflammation is activated when the mechanisms of acute inflammation fail to arrest infection or heal an injury. Left unchecked, prolonged chronic inflammation generates a series of destructive reactions that damages cells and eventually leads to clinical symptoms of disease.

Ultimately, chronic inflammation is a failure of the body's immune system to maintain a healthy homeostatic state.

Inflammatory Biomarkers include:
Obesity, Metabolic Syndrome, Type 2 Diabetes, Atherosclerosis, Cancer, and Rheumatoid Arthritis

Key Thought - Food Can Reduce Inflammation

Most persons with diabetes, high cholesterol, hypertension, and other chronic health problems have high levels of inflammation in their bodies that occur over time when the immune system tries unsuccessfully to repair cells and rid itself of harmful toxins.

The right foods can help reduce the amount of inflammation in the body and improve health. Here are some suggestions for clients and patients on ways to use clean eating to decrease inflammation:

1. Boost your intake of fresh fruits and vegetables. Aim to eat five to seven servings each of fruits and vegetables daily. Choose fruits and vegetables that are deep green, orange, yellow, and purple, since these have the greatest nutritional value. Ten to 14 servings per day may sound like too much, but serving sizes are small: one medium fruit, 1/2 cup frozen fruit, 1/2 cup cooked vegetable, 1/2 cup cut fresh fruit, and 1 cup leafy raw greens.

2. Cook without oil as much as possible saut� with water or veggie broth.. Make a quick and easy dressing using 1/4 cup balsamic vinegar, 1/2 clove minced garlic, spicy mustard, and 1 T each of chopped fresh parsley and chives. (Use 1/2 tsp dried herbs if fresh herbs aren't available.)

3. Snack on walnuts instead of chips. Walnuts provide fiber, minerals, antioxidants, and the kinds of fatty acids that are good for your brain and heart.

4. Eat whole grain cereals such as oatmeal or flax flakes for breakfast, and replace refined grains with whole grains, as in choosing brown rice over white rice.

5. Eliminate all animal products - meats, fish, milk, cheese, chicken and eggs.

6. Eliminate fast foods. Cook at home. Make meal time a family focus for health and renewal. Prep in advance, cook more to freeze some, and for lunches later in the week.

7. East more potatoes, especially sweet potatoes. They're high in vitamins and delicious when baked with a little sea-salt, pepper, garlic, and rosemary.

8. Cut out sugary drinks. Instead drink more water, sparkling water, home-juiced juices, herbal teas and green teas. With lemon wedges.

9. Eat more lentils and beans. They're good sources of protein and is a great replacement for red meat at meals. Try black beans and brown rice saut�ed with onions and garlic and seasoned with cumin.

10. Munch on dark chocolate and fresh raspberries for dessert. Both are loaded with antioxidants. Or try my favorite dessert, frozen bananas with berries and nuts.

If you make these simple changes, you'll lose weight, your skin will improve, you'll have more energy, sleep better, and life will continue to get better and better.

The Psychology of Hair Loss in Teenagers and Young Adults

In the late teens, an individual is dealing with pubertal and post pubertal physiologic changes, the challenges of seeing the world with a new eye, the loss of innocence, school, pressure form peers and social groups etc. These in themselves are psychological stressors that define and shape our humanity. The manner in which we cope with these early stressors have the potential for defining who we become for the remainder of our lives. Adding hair loss to that burden in a teenager's life can have far reaching consequences. There is potential for it to result in depression, social withdrawal and even paranoia. An individual whose development is influenced by these additional stressors would be at a disadvantage in all realms of life.

Hair Restoration Surgery In Teenage and Young Adults - What Could Go Wrong?

A young patient considering a hair transplant should always remember that transplanted hairs are permanent, while the hair they currently have is temporary as the hair loss would progress with age. Because the extent and rate of hair loss progression cannot be predicted, planning a hair transplant can be tricky. If this is not taken into account a hair transplant would potentially result in an odd hair pattern which results after a transplant that initially merged into adjacent pre-existing hairlines assumes an odd look. This would occur when the previously blended pre-existing hairline progressively receded to create a bald gap between it and the transplanted hairs. To correct this, more hair transplants would be required to bridge the gap. However, this odd pattern might not be remediable if:

1. The patient runs out of donor hair. This is especially a problem in patients with severe baldness, or when the transplanted hairline was placed too aggressively

2. The patient cannot financially afford the cost of further procedures

3. Body hair cannot be used either because the doctors at the patients' disposal lack the experience, skill or the technology required to perform successful FUE transplant using body hair (BHT) consistently. In some patients, however, BHT is not conceivable on account of an absolute lack of body and beard hair. There is a great deal of inter ethic variation in the quality and quantity of body hair in individuals.

Hair Transplant Repair Options for Victims of Hair Transplants Performed at Young Age

Unless the hair transplant on a young patient involved the creation of a very conservative hairline, the first challenge in most of these patients is the depletion of head donor hair supply often compounded by the presence of follicular unit strip surgery and / or FUE scars. In these head donor depleted patients, a successful and credible repair and restoration work would necessitate the use of beard and body hairs in    BHT repair work. Donor source in BHT can include facial hair as well as hairs from the torso and limbs. A recent 122 patient study published in the    Aesthetic surgery journal describes the criteria for determining the suitability of BHT donor hair. What more, the study described the kinds of features an FUE equipment used in BHT should possess. Younger patients destined for severe baldness should begin incorporating beard and body hair earlier on in their hair transplantation odyssey. With proper planning, requisite doctor experience and skill as well as the proper utensils it is indeed possible to execute a successful FUE repair in young victims of ill-conceived hair restoration surgeries that were performed at a young age.
